    U.S. World Cup team Head Coach Steve Sampson's decision on who to start at forward did not become any clearer today, as the U.S. failed to score for the second time in its last three exhibition matches. Though the U.S. created a number of scoring chances in its 0-0 tie against Scotland, they failed to convert any of them.     Debbie Keller scored twice and Lorrie Fair tallied her first international goal as the U.S. women's national team crushed New Zealand, 5-0, at RFK Stadium May 30.     MLS produced a videotape showing the worst of soccer in hopes of creating a better game. It's called "Sending-Off Offenses" and my legs are sore just from watching it. It comprises 28 brutal tackles from 1997 MLS action.
